L’étude ANRS CO12 CirVir a pour but de décrire l'histoire naturelle d'une vaste cohorte de patients atteints de cirrhose virale.

L’essentiel

Entre 2006 et 2017, la cohorte ANRS CO12 CirVir a suivi plus de 1 800 patients atteints de cirrhose virale due à une hépatite B et/ou C.

L’objectif de CirVir était d’identifier les facteurs associés à la survenue des principales complications cliniques, d’évaluer le niveau de risque de ces complications et ainsi d’adapter la prise en charge des patients en fonction du niveau de risque de chaque complication.

Les données collectées ont également permis le développement d’une base de données et leur intégration dans divers programmes de recherche internationaux.

La cohorte ANRS CO12 CirVir comporte le suivi prospectif de près de 1 800 patients atteints de cirrhose virale inclus dans 35 centres répartis sur l’ensemble du territoire Français et suivis depuis 2006.

L’objectif de la cohorte CirVir

Si plusieurs cohortes se sont attachées à décrire l’évolution des patients atteints d’une infection virale B et/ou C, très peu d’entre elles ont étudié le devenir de ces patients lorsque la maladie hépatique avait atteint le stade de cirrhose ou l’incidence de la survenue du carcinome hépatocellulaire. La cohorte ANRS CO12 CirVir s’inscrit dans cet objectif.

L’étude ANRS CO12 CirVir a pour but de décrire l’histoire naturelle d’une vaste cohorte de patients atteints de cirrhose virale non compliquée suivis de façon prospective, d’identifier les facteurs associés à la survenue des principales complications cliniques, et d’identifier et valider des scores prédictifs permettant d’évaluer le niveau de risque de ces complications.

Critères d’inclusion et suivi

Entre mars 2006 et juin 2017, 1 822 patients ont été inclus dans les 35 centres participants.

Les patients ont tous une cirrhose, non compliquée et histologiquement prouvée ayant au moins pour origine une infection par le virus de l’hépatite B et/ou C. Ils sont suivis prospectivement, selon les recommandations largement établies, en termes de consultation spécialisée, d’examen de morphologie hépatique (échographie, IRM, TDM) et de bilan biologique tous les 6 mois.

De plus, des prélèvements biologiques seront effectués à l’inclusion et tous les 12 mois afin de constituer une sérothèque, une plasmathèque et une DNAthèque (concentration de cellules nucléées).

Une expertise en gestion et exploitation de biobanques

L’exploitation des données de la cohorte ANRS CO12 CirVir au sein de l’unité de recherche clinique (URC) de l’Hôpital Henri-Mondor a permis le développement d’une base de données commune avec d’autres cohortes françaises et européennes, issues ou non des programmes de l’ANRS MIE (cohorte ANRS CO22 Hepather, projet ANRS LICAVIR, cohorte APHP CIRRAL, essai APHP CHC 2000), regroupant désormais plus de 5 000 patients avec un suivi longitudinal prolongé. « Cette mutualisation des données constitue un outil unique pour l’étude des complications de la cirrhose, et en particulier du carcinome hépatocellulaire, en permettant des analyses à grande échelle et l’identification de biomarqueurs pronostiques robustes » déclare l’équipe du projet.

Les travaux de la cohorte CirVir ont engendré l’acquisition d’une véritable expertise de l’équipe du projet en matière de gestion et l’exploitation de biobanques séquentielles associées aux données cliniques longitudinales, notamment grâce au soutien de l’ANRS MIE sur des financements dédiés (projets CITHEC et STHEPBIO-virus).

Cette expertise a entraîné l’intégration de la cohorte CirVir dans plusieurs projets structurants à forte visibilité et valorisation industrielle, parmi lesquels le projet BPI HECAM en 2018, le RHU DELIVER en 2022, le projet européen GENIAL en 2023, ainsi que le RHU LIVERTRACK en 2024.

Améliorer la prise en charge des patients à risque

Les résultats de cette étude doivent permettre d’adapter la prise en charge des patients en fonction du niveau de risque de chaque complication : dépistage et traitement des lésions dues à l’hypertension portale, dépistage et traitement précoce du carcinome hépatocellulaire (CHC), chimioprévention du CHC, indications à la transplantation hépatique, etc.

Plus de 30 publications, dont plusieurs articles parus dans des revues internationales de tout premier plan telles que Journal of Hepatology, Hepatology, Gut et Gastroenterology, sont issus des résultats de la cohorte CirVir. « Ces travaux ont contribué de manière significative à l’amélioration des connaissances sur l’histoire naturelle de la cirrhose virale, ses complications et les facteurs pronostiques associés » affirme l’équipe du projet.
